Call 413-731-7877 to arrange an appointment or request a consultation during regular office hours; Monday - Friday 8:00 AM to 4:30 PM. For medical emergencies, dial 911 or call the emergency services number for your area.

Consultation Fee Discount

When you book a consultation, there is a $100 consultation fee. If you choose to have surgery with Dr. Johnson the $100 will go towards your surgical fee.


Cancellations

We’ll contact you prior to your appointment date to confirm the time still works for you. If you need to cancel or change your appointment, please let us know. Cosmetic (pre-paid) consultation cancellations are refundable at the following rates:

  • Cancellations greater than 48 hours before the appointment are eligible for a 100% refund of consultation fee.
  • Cancellations less than 48 hours but greater than 24 hours before the appointment are eligible for a 50% refund of consultation fee.
  • Cancellations less than 24 hours before the appointment and no-shows are not eligible for a refund of the consultation fee.

New Patient Pre-Consultation Forms
